This resource includes strategies, tips and tools that leaders can use to guide their practices through PMH transformation. Key changes for engaged leadership include the following:
- Provide visible, sustained leadership to guide overall culture change, as well as specific strategies to improve quality and spread and sustain change.
- Ensure that the PMH transformation effort receives the time and resources needed to succeed.
- Ensure that care providers and other team members have protected time to conduct activities beyond direct patient care that are consistent with the medical home model.
- Build the practice's values relating to creating a medical home for patients into staff hiring and training processes.
