Osteoarthritis Tool

Description:

The Osteoarthritis Tool was developed for primary care providers managing patients with new or recurrent joint pain consistent with osteoarthritis of the hip, knee or hand. This tool can help clinicians identify symptoms and provide evidence-based, goal-oriented non-pharmacological and pharmacological management while identifying triggers for investigations or referrals.

Osteoporosis and Falls Assessment Form

Description:

An osteoporosis and falls assessment tool based on the 2010 Clinical Practice Guidelines for the Diagnosis and Management of Osteoporosis in Canada and the 2015 Clinical Practice Guidelines for the Frail Elderly, that can be integrated into the electronic medical records to support improved osteoporosis-related care in family practice.

Ottawa Ankle Rules - for Ankle Radiography

Poverty: A Clinical Tool for Primary Care Providers

Poverty puts patients at higher risk for most chronic diseases, mental illnesses, and even accidents and trauma. The Tool directs providers to use key questions to assess their patients’ living situations and current benefits and includes links to key government and community resources to support positive interventions.

Preconception Health Care Tool

The Preconception Health Care Tool helps you assess the preconception health and risk factors for pregnancy of all patients of reproductive age. Use it over a series of visits to identify potential physical, genetic, psychosocial and environmental risk factors for adverse pregnancy outcomes. The Tool will guide you to help reduce those risks before conception through counselling, education and other interventions.
